How to Open an ALF in Madison County, Florida

When Carline contacted the Florida Department of Health in Madison County in 2025 for local group-care guidance, the response was essentially a referral to the statewide Residential Group Care program. That means an operator cannot expect one county handout to organize the entire project.
In Madison County, due diligence must be assembled across the correct land-use jurisdiction, building and fire review, water and wastewater, Environmental Health, emergency management, and AHCA. The place to begin is still the property, but the first property question is whether you are inside the City of Madison or in unincorporated county.
City and County Are Separate Approval Front Doors
Madison County Planning and Zoning administers the Land Development Code and Comprehensive Plan for unincorporated property. The county's current Madison County Land Development Code page states that its provisions apply to all land, structures, buildings, and uses in unincorporated Madison County and that development cannot be undertaken without prior authorization under the code.
Inside the City of Madison, City of Madison Planning and Development provides its own planning, zoning, development, building permitting, and code-enforcement services. So do not use a Madison mailing address as your jurisdiction test. Confirm whether the parcel is actually municipal before you rely on the county path.
When No ALF-Specific Rule Is Easy to Find, Ask a Better Question
I did not find a current county webpage that gives a clean, ALF-specific zoning formula for every resident count and district. I would not fill that gap with a guess. Give Planning the parcel number, existing use, proposed licensed capacity, and the fact that it will be an AHCA-licensed assisted living facility, then ask for a written determination of how the use is classified and what approval is required.
That answer should come before renovation plans. A house can be physically attractive and still have land-use, access, parking, prior-permit, or occupancy issues that make it a poor ALF candidate.
The Health Department Still Matters, Even Without a Local Checklist
The internal Madison response pointed Carline to the statewide Residential Group Care program rather than issuing a county-specific list. Current statewide DOH rules remain important: physical-plant sanitation is part of the group-care review, and facilities with 11 or more beds must obtain a separate food-hygiene permit and complete food-service plan review.
Water and sewage also need a real answer. For public systems, confirm service and capacity with the provider. For septic or private water, determine the current DEP/DOH review path for that property instead of relying on the fact that the residence has historically functioned as a home.
Building and Fire Review Can Redefine the Renovation Budget
A residential property is not automatically a compliant assisted living facility. Before committing to cosmetic work, ask what the proposed occupancy and capacity trigger for building and fire/life-safety review. Legal exits, bedroom layouts, ramps, bathrooms, smoke detection, electrical load, generator placement, and unpermitted additions can all affect cost.
I would rather know those facts while the purchase is still subject to due diligence than after the business has already paid for furniture and licensing work.
Emergency Planning Needs a Local Confirmation
Madison County Emergency Management is the local emergency-management office. Florida's ALF CEMP rule requires coordination with local emergency management, but I did not find a detailed Madison-specific healthcare facility submission page. Confirm the current intake method, supporting documentation, and review expectations directly with the county.
Build the Madison File From Multiple Verified Answers
Madison County is not a county where I would wait for one agency to hand me the whole roadmap. I would create the roadmap myself: written land-use determination, jurisdiction, building/fire feasibility, utilities, Environmental Health, emergency-plan coordination, then AHCA licensing.
That is also why the thin local health-department response matters. It tells you to stop looking for a single magic checklist and start managing the project as several approvals that all have to agree with the same property.
