How to Open an ALF in Okeechobee County, Florida
Oct 05, 2026Okeechobee County is a good example of why a smaller market should not be mistaken for a simpler ALF project. The approvals may involve fewer layers than a large metro county, but the property still has to work under local land-development rules, building and fire requirements, utility conditions, and a very specific emergency-plan review process.
I would also price the local development costs before deciding what a property is worth. Okeechobee County currently publishes an assisted-living impact-fee category calculated by bed. That is the kind of cost that can materially change a project when you move from a six-bed concept to a larger facility.
Separate County Property From City Property First
Okeechobee County Planning and Zoning handles the unincorporated county and directs applicants to current zoning, Future Land Use, flood-hazard mapping, and the county Land Development Regulations. If the property is inside the City of Okeechobee, do not use the county process by default.
For the actual parcel, I would confirm the zoning classification, Future Land Use, whether the proposed ALF is permitted or needs additional land-use approval, and whether a development or special-exception process applies. The county's Planning Board handles special exceptions, variances, and major-development site plans, so the approval path can become a public process depending on the proposal.
Put the Per-Bed Development Cost Into the Property Math
Okeechobee County's current impact-fee page lists Assisted Living as a bed-based land-use category. I would recheck the amount immediately before budgeting, but the important point is that resident capacity can affect local development cost, not only staffing and AHCA fees.
That means capacity should be decided before the financial model is finished. If you are comparing two properties, the better-looking house is not necessarily the cheaper ALF once development fees, conversion work, fire requirements, parking, septic, or utility upgrades are included.
Do Not Underestimate the CEMP Lead Time
Okeechobee County's Healthcare Facility CEMP Review page currently says the county does not use an electronic emergency-plan review system. Plans are submitted as a tabbed binder or a single electronic document, and the county advises a minimum 90-day lead time for review.
That is not something I would leave until the end. Build the CEMP while the facility is being prepared, make sure transportation and receiving arrangements are real, and confirm the current submission instructions before filing.
The Property Still Has to Pass the Physical Tests
The county's planning page notes that building, fire, and Health Department inspections may be required as part of opening a business. For an ALF, I would test egress, accessibility, bedroom use, electrical and generator capacity, water and sewage, and any change-of-use issues before treating the property as ready.
Confirm Water, Sewer, and Septic Capacity Early
Okeechobee is still a rural market in many areas, so do not assume a property has public water and sewer simply because it looks developed. If the site relies on a private well or septic system, remember that most Florida counties transferred onsite sewage permitting from the local health department to the Department of Environmental Protection in 2025. DOH-Okeechobee can still tell you whether that applies locally and whether the existing system was sized for a single-family home rather than a licensed care facility. A system built for a family of four will not necessarily support a six-bed or larger ALF without modification, and that modification has its own permitting timeline.
Put the Contingencies in the Contract
So much of the Okeechobee math depends on answers you do not have yet that I would not sign a purchase contract without a due-diligence period built around them. Ask for enough time to get the zoning and Future Land Use answer in writing, confirm the current impact-fee figure, have the septic and water evaluated, and walk the building with someone who knows fire and egress. If the seller will not allow enough time for those steps, the discount you think you are getting may already be spoken for by the work you will have to do later.
Count Backward From the 90-Day CEMP Review
The CEMP lead time is the one number on this list that you cannot negotiate. If the county advises at least 90 days, your earliest realistic opening date is not the day construction ends. It is ninety days after a complete plan goes in. I would draft the plan around the actual property, line up the transportation and receiving agreements early, and ask the county what a complete binder or electronic file looks like so nothing comes back for formatting. Work backward from that date, and the rest of the schedule gets much more honest.
